Currently, no CPT code exists that describes the full work performed in a hi

p core decompression. The American Medical Association’s CPT guide states that this procedure should be reported using the unlisted code, 27299. There are several codes, however, that could represent components of this procedure.

Review the operative report thoroughly for appropriate code assignment. In a

partial hip replacement (81.52), also called a hemiarthroplasty, only the femoral head is replaced and the acetabulum is left in place. A partial hip replacement is done mainly to repair fractured hips.

Background: Many orthopedic practices routinely code hip fracture hemiarthro

plasty as Current Procedural Terminology (CPT) 27125 even though 27236 is the correct CPT code. Our objective is to determine the financial impact this simple mistake has on surgeon reimbursement.

Action: The first option is to report this using code 27006 (Tenotomy, abduc

tors and/or extensor (s) of hip, open (separate procedure)). The second option is to use code 27062 (Excision; trochanteric bursa or calcification) along with code 27299 (Unlisted procedure, pelvis or hip joint).
