Did you figure this out? The symptoms you describe seem identical to mine. I

get E012 flame loss for a similar Navien combo-boiler. It heats up and seems to do fine for I short time. But only in heating mode, not DHW, then loses flame (but not always). I finally had someone out who had Navien on the phone.

Navien has refused to help and their customer service is as useless as their

manual. When I call they tell me what the codes, word for word in the manual book. They offer no solution. The Boiler is garbage and they refuse to honor the warranty.Desired Settlement: I want Navien to send a certified pro they have on record to fix this issue.

The Navien is in the basement, the recirc line runs east from the Navien to

the kitchen then up to the 2nd floor, then west across the 2nd floor, then back down to the master bath shower. I have 2 theories, the water is cooling down in the loop causing the temp in the recirc line to cause fluctuations at the master shower.
